Participation in a vocational functional assessment

Participation in Service Plan Development

Participant is working independently in the community

Participant is still able to receive services as determined by the
participant and the Career Services Center Staff after exiting the program
INDEPENDENT/SUPPORTED
HOUSING
ENTRANCE

Demonstrate ability to self-advocating
·
Self-administer medication
·
Able
to complete ADL’s with minimal assistance
·
Willing to participate in initial skills assessment to identify need
areas
·
With
training, able to complete Activities of Daily Living independently within 6
months to one year

Complete all skills on entrance skills assessment 90% of the time with 90%
accuracy

Has
obtained housing in the community
