DROP-IN CENTER

ENTRANCE

·       Any individual that is diagnosed with a major mental illness

EXIT

·       None

 

CAREER CENTER

ENTRANCE

·        Any individual that is currently receiving services from NEFSH Community Behavioral Healthcare Services

·        Interested in obtaining employment (volunteer, enclave, supported, transitional, or independent)

·        Referred through NEFSH Community Behavioral Healthcare Services

·        Participation in a vocational functional assessment

·        Participation in Service Plan Development

EXIT

·        Participant is working independently in the community

·        Participant is still able to receive services as determined by the participant and the Career Services Center Staff after exiting the program

 

INDEPENDENT/SUPPORTED HOUSING

ENTRANCE

·        Demonstrate ability to self-advocating

·        Self-administer medication

·        Able to complete ADL’s with minimal assistance

·        Willing to participate in initial skills assessment to identify need

areas

·        With training, able to complete Activities of Daily Living independently within 6 months to one year

EXIT

·        Complete all skills on entrance skills assessment 90% of the time with 90% accuracy

·        Has obtained housing in the community

·      Demonstrated ability to maintain personal budget

 

Expectations of Members

 

v           Please be respectful of others and their belongings

 

v           Please smoke only in designated areas

 

v           Please attempt to solve disagreements as independently and as quickly as possible

 

v           Please speak in a respectful tone and refrain from yelling or swearing

 

v           Please refrain from any physical aggression

 

v           Please remember, “If you bring it, take it with you!”

 

v           If you mess it up, please clean it up!

 

v           HAVE FUN!!!
